1
Letter of Medical Necessity
This is required to prove that the expense is for the treatment of a diagnosed condition.
2
Detailed Receipts or Invoices
Detailed payment records, including date of service, description of service, provider, and amount paid.
3
Claim Form (Required for Reimbursement)
Most administrators require their own version of a claim form for reimbursement. We can help you submit this information.
